Short answer: Religion can be “None”. Issuing authority is the authority printed in your passport, or the issuing country if there is none. Contact address can repeat your home address. Fill in every field, even the ones that look optional, because blank fields are a common reason the form will not submit.
The rule for every field: copy what your passport says, in the same spelling and order, without accents. When the question is about you rather than your passport, a short, honest answer is always enough.

Religion
It is a required text box, and it is there for record keeping. Write one word. If you do not follow a religion, “None” is a normal answer. There is no “right” option and nothing suggests the answer affects the decision.
Passport issuing authority (place of issue)
This is the field people guess most often. Open your passport at the photo page and look for a line called Authority or Issuing authority. Copy that text. A US passport, for example, prints “United States Department of State”.
Some passports do not have an authority line at all. In that case write the country that issued the passport. Do not write your city of birth here, and do not leave it empty.
Permanent address vs contact address
Permanent address is where you live. Contact address is where you can be reached, which for most people is the same place, so you can repeat it. If you live abroad, use the country you live in now, not the country on your passport.
Emergency contact
Pick a family member or friend at home, not the person sitting next to you on the plane. Give their full name, address, phone with country code and the relationship (mother, brother, friend).
Address in Vietnam: why the ward list may not match
Vietnam reorganised its provinces and wards in 2025 and removed the district level. Many hotel listings still show the old address with a district and an old ward name, so the dropdown on the form may not show the ward you are looking for.
- Type the hotel address in the text box as the hotel gives it.
- For the city and ward dropdowns, pick the new name, not the nearest-looking one.
- Not sure which ward it is now? Message the hotel and ask for their current ward “for an eVisa form”. They get this question often, and their answer is more reliable than an old listing.
The address is not binding. You can stay somewhere else later, but it has to be a complete, real address.
Agency, organisation or person to contact in Vietnam
This section is meant for business trips and sponsored visits. For a normal holiday, your first hotel is a sensible answer, or your tour company if you have booked one. Business travellers should enter the company that invited them, as explained in our business visa guide.
The yes or no questions
Questions about other passports, multiple nationalities, previous visits and violations of Vietnamese law are simple: answer truthfully. A previous visit or a second nationality is not a problem in itself. Giving a false answer is.

Can I leave the optional fields empty?
You can try, but empty fields often trigger the “complete all the blanks” message. Filling them in takes a minute and avoids the problem. See that error and its fix.
I entered the wrong issuing authority. Is that a problem?
Usually not, as long as the passport number, name, dates and nationality are correct. Those are the fields that must match exactly. If one of those is wrong, read how to fix a mistake.
What if I have not booked a hotel yet?
Pick a hotel in your arrival city and copy its complete address. Entering it on the form does not book a room or commit you to staying there.
Do I need to write my religion in English?
Yes, use English and plain letters without accents, like every other field.
